Doctor of Medicine (MD) at Universiti Sains Malaysia (USM) is a five-year undergraduate medical programme delivered through an integrated and system-based curriculum. The programme is structured into two phases: Phase I (Preclinical) in Years 1 and 2, and Phase II (Clinical) in Years 3 to 5, ensuring a seamless transition from foundational sciences to clinical practice.
The curriculum is guided by the SPICES model, which emphasises Student-centred learning, Problem-based learning, Integration, Community-based education, Electives, and a Spiral and Systematic approach. This model promotes active learning, critical thinking, and the application of medical knowledge in real-world and community settings. To meet evolving healthcare demands and technological advancements, SPICES 2.0 is incorporated to prepare future-ready physicians.
Implemented through a credit-unit and semester-based system, the programme upholds USM’s motto, “Compassionate Professional,” nurturing doctors who combine clinical competence with empathy, ethical practice, and a strong commitment to patient, family, and community well-being.

Year 1 - Year 2
Phase 1 is a pre-clinical years which is consist of Year 1 and Year 2 in medical programme. The students will learn basic sciences and basic medical courses.
Year 3 - Year 4 - Year 5
From Year 3 to Year 5, students enter the clinical phase of the medical programme. During this phase, students undertake core clinical courses, participate in the Community and Family Case Study (CFCS) programme, and complete clinical electives in healthcare settings such as hospitals and clinics. They are also involved in community and health district visits, while receiving extensive hands-on exposure to ward work and the management of real patients under clinical supervision.
